Medical Cannabis Certification Eligibility
The Florida Statutes listed in Chapter 381 outline the general provisions of Medical Use of Marijuana. A patient must be diagnosed with at least one of the following to receive medical cannabis or a cannabis delivery device:
(a) Cancer
(b) Epilepsy
(c) Glaucoma
(d) Positive status for HIV
(e) AIDS
(f) Post-traumatic stress disorder
(g) Amyotrophic lateral sclerosis
(h) Crohn’s disease
(i) Parkinson’s disease
(j) Multiple sclerosis
* Medical conditions of the same kind or class as or comparable to those enumerated in (a)-(j).
A terminal condition diagnosed by a physician other than the qualified physician issuing the physician certification.
Chronic nonmalignant pain - "caused by a qualifying medical condition or that originates from a qualifying medical condition and persists beyond the usual course of that qualifying medical condition"
* This application does not guarantee that you will receive a cannabis certification or constitute a physician-patient relationship. Only a certified physician during an in-person exam and after reviewing your medical history can determine if you qualify.
* Florida law allows doctors to certify patients with medical conditions of the same kind or class as or comparable to those enumerated in (a)-(j). When debilitating, these may include but are not limited to:

Florida Cannabis Clinic Certification Process
Step 1
Complete Survey
Fill out the eligibility survey and create an account on our site. This should only take a couple minutes.
Step 3
In-Person Exam
Supporting medical documents are required. If you qualify, you will be entered into the Medical Marijuana Use Registry.
Step 2
Book Appointment
Book appointment online and complete intake forms emailed to you. Please note a credit card is required to hold your exam with the physician.
Step 4
Get Your Card
Login to Registry & Complete DOH online application to receive medical cannabis card in about 1-2 weeks.

Medical cannabis in Florida is cultivated by DOH approved treatment centers in a secured, regulated, and controlled environment.
Raw material and essential compounds are harvested from the cannabis plants. These components are then laboratory tested to ensure safety and effectiveness. Treatment centers may openly provide Certificates of Analysis for quality testing.
Medical cannabis that is approved by independent testing labs is formulated into various routes of administration including inhalation, oral, topical, sublingual & rectal preparations.
